In an effort to help you find your way through the health care system, here is a brief overview of the key agencies you will come across. These agencies can provide you with the necessary support and guidance regardless of the stage of the disease.

The Alzheimer Society provides support and information to individuals and their families who are living with or concerned about Alzheimer's disease and other dementias in Ottawa & Renfrew County. The services provided are listed below. Please call for details about times and locations of events that are of interest to you.

First Link Program
This program links individuals newly diagnosed with dementia and their families to a community of learning, services and support. In collaboration with the Champlain Dementia Network, First Link offers peer support and a progressive learning series. The learning series include: Next Steps, Care Essentials, Options for Care and Care in the Later Stages. Sessions are available in French. For further information click here

Family Support & Education Services
Our family support & education team can provide bilingual services in the form of information, support and education to families who contact the Alzheimer Society of Ottawa
and Renfrew County in person, over the phone and in caregiver groups. Call them from Monday to Friday, 8:30 a.m. to 4:00 p.m. at 613-523-4004.

Monthly Support Groups
Support groups offer a confidential, compassionate and knowledgeable environment where individuals can discuss challenges they encounter in everyday living with dementia. Education and information sharing is an integral part of these support group sessions. We offer:
• English and French Family Support Groups
• Wives Support Groups
• Husbands/Sons Support Groups
• Daughters Support Groups
• “Especially for You” Support Groups for individuals in the early stages of dementia
• Caregiver Support Groups for carers of those in the early stages of dementia

• Caregiver Support Groups in collaboration with Day Programs
• “Between Us” Support Group for persons with Early Onset dementia (persons under the age of 65 years with a diagnosis of dementia)
• Early Onset Caregiver Support Group (caregivers of persons with dementia under the age of 65 years)
• “Morning Break” Caregiver Support Group in Metcalfe
• Koffee Klatsch: a social group for individuals in the early stages of dementia and their carer (in French & English)
• Info Rencontre: Caregiver information/discussion session
Upcoming Events – Support Groups

Education Workshops
Two hour workshops are held once a month, designed for family caregivers on topics relating to various aspects of caregiving. Topics such as communication, challenging behaviours, coping strategies, and ethics are presented. Resources for future reference are shared. Upcoming education workshops.

Specialized Workshops
Workshops are held at various times throughout the year on topics such as: After Placement, Bereavement, and Autopsy. Learning sessions are offered to Family Councils in long-term care homes, to family members in retirement homes, and facilitator workshops are offered to volunteers leading support groups.

Speakers' Bureau
Presentations on various aspects of Alzheimer's disease and related disorders as well as the programs and services of the Alzheimer Society can be given to community organizations.

Newsletter
Our chapter’s quarterly newsletter, Alzheimer lifeline, provides information on: caregiver tips, research, local news, educational programs, etc. Alzheimer lifeline.

Alzheimer Info Line
A telephone information service that consists of short messages on topics such as symptoms of Alzheimer's disease and how to obtain a diagnosis, and the Safely Home™ - Alzheimer Wandering Registry. Operating outside office hours (evenings and weekends), callers can access this service by calling our office at 613-523-4004 on a touch tone phone. One message is dedicated to news on upcoming meetings, support groups and activities of interest to Alzheimer families.

Volunteer Education Program
This program is offered to adult and/or student volunteers who find themselves working with residents/clients who have Alzheimer's disease or a related dementia.

Enhancing Care Program
An assessment program in long term care facilities and community programs facilitated by the Alzheimer Society of Ottawa
and Renfrew County which assists in measuring the effectiveness in enhancing care for people with dementia. It is based on Alzheimer Society of Canada’s booklet Guidelines for Care.

Satellite Office in Renfrew County
Caregivers in Renfrew County can now access support and education through the Pembroke and the Renfrew offices. Support groups are held monthly in Arnprior and Deep River and portable libraries can be accessed at Pembroke, Renfrew, Barry’s Bay, Deep River, Beachburg, Arnprior, and Eganville. Speakers’ Bureau presentations are available. Volunteers are always welcome. For more information, please call 613-732-1159 in Pembroke (Mon. & Tues.) and 613-433-5896 in Renfrew (Wed. & Thurs.).

Safely Home™ – Alzheimer Wandering Registry
Developed by Alzheimer Canada with the RCMP, this program is designed to help find the person with Alzheimer's disease or a related dementia who is lost and assist in a safe return. Registration is voluntary. When registered, vital information about the individual is stored confidentially on a police database. This information can be accessed by police anywhere in Canada. For a one time fee of $35, an identification bracelet, a Caregiver Handbook and ID cards are provided. Registration forms are available at our Ottawa, Renfrew and Pembroke offices.

Resource Centre
Books and videos on Alzheimer topics such as coping techniques and strategies, activities for the Alzheimer person, or placement issues are available to help both family and professional caregivers. Fact sheets, Alzheimer journals and other reference materials are also available.

In home services
The CCAC provides, pays for and coordinates professional health and treatment programs for clients in their homes. Services include visiting nursing, physiotherapy, occupational therapy, speech therapy, and related professional and support services. A case manager assesses the eligibility of clients based on criteria established by the Ministry of Health and arranges for services that address the needs identified by the case manager, client and/or caregiver. This service offers:

• Up to 60 hours per month of free personal support assistance based on client/caregiver needs
• Limited respite hours to those who meet criteria
• Case managers to support individuals and families living with dementia

You can apply for in-home care if the person you are caring for:

• Lives in Ontario and has a valid Ontario Health Card
• Cannot get to a clinic or hospital for the care that she needs
• Can be cared for safely at home

Long-term care admission process
The CCAC also manages the process for long-term care facility admission in the city. This includes permanent residency as well as subsidized short-stay respite beds. This does not include retirement homes.

Information and referral
The CCAC will tell you about, or put you in touch with, other groups and services in the region that have the help you need. If you want to access services from CCAC, call and ask to speak to an intake worker.

The service’s main focus is individuals who have been diagnosed with moderate to severe dementia.

Dementia counselors
They will work with the individual and family/caregiver in his home to provide functional assessment, education, counseling, behavioural management and advice regarding community services and future planning. Assessments for personal/property capacity are also available for a fee.

Consultations with geriatric psychiatrists
They are available upon request if the individual is depressed or if there are problems with behaviours or other psychiatric problems. This may include a home visit, if necessary. (A physician referral may be required for this service.)

This service focuses on the early diagnosis (mild to moderate) of dementia and the difficult diagnosis of atypical patients with dementia at any stage and at any age. A physician referral is required.

Both of these programs focus on the diagnostic assessment in people with dementia and other medical problems in people over the age of 60. A physician referral may be required.

Geriatric Assessment Outreach Teams
Health care professionals from various backgrounds (multidisciplinary team) visit individuals in their home to identify problems and refer to other RGAP or community programs.

Geriatric Day Hospitals
This hospital-based program provides a multi-disciplinary team who assesses and diagnoses problems and provides treatment and rehabilitation services on an outpatient basis.

Royal Ottawa Hospital Geriatric Psychiatry Services

This program provides treatment to those with dementia who also have psychiatric problems such as depression or behavioural problems. Assessments are initially provided either at the Royal Ottawa Hospital on an outpatient basis or in long-term care facilities as part of its outreach services. After an initial assessment, treatment may be provided in the Geriatric Psychiatry Day Hospital or inpatient service. A physician referral is required.
