Ontario
Trillium Foundation Grant for the Champlain Dementia Network:
The
Ontario Trillium Foundation is providing $85,000 funding to the
Alzheimer Society of Ottawa
and Renfrew County over two years (2006
– 2008) to assist the Champlain Dementia Network in its
efforts to improve access to caregiver support and ensure better
co-ordination of service delivery to persons with dementia and
their family members in Eastern Ontario.
In
particular, the Champlain Dementia Network will develop and implement
a web-based services mapping inventory for the Champlain region
which will be easily accessible and user friendly to support persons
with dementia and their caregivers. The Network will also complete
a strategic planning process which will include the services mapping
inventory, a gap analysis, the identification of key strategic
directions and goals for one, two and five-year timeframes. It
will also develop and implement action plans for several of the
one year and two year goals.
In
the course of undertaking the above, it will work in collaboration
with other networks as appropriate.
Alzheimer
Strategy Transition Fund, Ontario Ministry of Health and Long
Term Care: The Champlain Dementia Network is grateful
to the Alzheimer Strategy Transition Fund for a grant to support
in part the development of the CDN web-based services mapping
inventory including the development of the French infrastructure
to ensure that the site provides bilingual access.
Community
Foundation of Ottawa: The Champlain Dementia Network
is appreciative that the Community Foundation of Ottawa awarded
the Network with a grant for the French translation of the web-based
services mapping project for dementia care in the Champlain region.
This project will have far reaching benefits not only for those
in the Champlain region but also to the French speaking families
anywhere in Canada and beyond who may have relatives in the Champlain
region.
